MVA - Motor Vehicle Accident

If you have been in a motor vehicle accident, you may be eligible for services paid by your insurance provider.

 

If you have been in a motor vehicle accident in Alberta and have suffered an injury, please proceed with the following steps:

1) Contact your automobile insurance company to inform them of the incident and receive further instruction.

2) Complete an AB-1 Proof of Loss Form, which can be downloaded here under "Accident Benefit Claim - prescribed forms": Download AB-1 Form. You may also complete an AB-1 Form through our Jane App system, which will be automatically sent to you after you book and MVA Initial Assessment with one of our physiotherapists.

3) Book an "MVA Initial Assessment" appointment with one of our physiotherapists. Please bring the following documents or information to your initial assessment appointment:

  • Copy of AB-1 Proof of Loss Form
  • Name and contact information for your Insurance company
  • Name and contact information for your Insurance adjuster

4) Receive physiotherapy and potentially massage therapy treatment at One Wellness depending on approval by your insurance adjuster.

In the event your claim is declined by your insurance company, you are responsible for payment to One Wellness for any services and/or products provided.

 

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the practitioners' day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment.

Patients accessing MVA direct-billed services who provide 12-24 hours notice, will be charged a cancellation fee equal to 50% of the cost of a regular physiotherapy appointment.

Patients accessing MVA direct-billed services who provide less than 12 hours notice, or miss their appointment, will be charged a cancellation fee equal to the cost of a regular physiotherapy appointment.

Late cancellation or no show fees CANNOT be billed to your insurance company, and will be charged automatically to the card on file.

All clients/patients are required to have a card on file to uphold our clinic cancellation policy.
