Your Privacy is Important to Us

 1. Our Commitment to Privacy

One Wellness is committed to protecting the privacy, confidentiality, and security of personal and health information entrusted to us by our patients. We collect, use, and disclose personal and health information in accordance with the Health Information Act (HIA), the Personal Information Protection Act (PIPA), and the standards and guidelines of the College of Physiotherapists of Alberta (CPTA).

2. Definitions

  • Personal Information: Information that identifies an individual, such as name, contact details, date of birth, or billing information.
  • Health Information: Information about an individual’s physical or mental health, health history, assessments, diagnoses, treatment plans, and progress notes.
  • Custodian: One Wellness and its regulated health professionals, acting as custodians of health information under Alberta legislation.

3. Collection of Information

We collect only the information necessary to provide safe, effective, and high-quality care. This may include:

  • Personal identification and contact information
  • Health history and current medical information
  • Assessment findings, treatment plans, and progress notes
  • Insurance and billing information
  • Appointment and attendance records

Information is collected directly from patients whenever possible, or from other sources with patient consent or where permitted by law.

4. Use of Information

Personal and health information is used for purposes including:

  • Providing assessment, treatment, and ongoing care
  • Communicating with patients regarding appointments and care
  • Billing, payment processing, and insurance claims
  • Quality assurance, risk management, and professional compliance
  • Meeting legal, regulatory, and professional obligations

5. Disclosure of Information

We do not disclose personal or health information without patient consent, except when:

  • Required or permitted by law
  • Necessary for continuity of care (e.g., referrals, reports to other healthcare providers)
  • Required for billing or insurance purposes
  • Required by regulatory bodies, including the CPTA
  • There is a risk of serious harm to the patient or others

Only the minimum amount of information necessary is disclosed.

6. Consent

By receiving services at One Wellness, patients provide informed consent for the collection, use, and disclosure of their information as described in this policy. Patients may withdraw or limit consent at any time, subject to legal and professional obligations.

7. Safeguards and Security

We take reasonable steps to protect personal and health information against loss, theft, unauthorized access, disclosure, copying, or modification. Safeguards include:

  • Secure electronic medical record systems
  • Password-protected computers and devices
  • Locked filing cabinets for paper records
  • Staff training on privacy and confidentiality
  • Restricted access to information on a need-to-know basis

8. Retention of Records

Health records are retained in accordance with Alberta legislation and CPTA requirements. Records are securely destroyed once retention requirements are met.

9. Access and Correction

Patients have the right to:

  • Request access to their personal and health information
  • Request corrections to inaccurate or incomplete information

Requests must be made in writing and will be handled in accordance with Alberta legislation.

10. AHS-Funded Physiotherapy

One Wellness will notify Alberta Health Services (AHS) without undue delay of any actual or suspected privacy breach involving information associated with AHS-funded physiotherapy services and will comply with all applicable AHS policies, contractual obligations, and legislative reporting requirements.

12. Policy Review

This Privacy Policy is reviewed regularly and updated as required to ensure ongoing compliance with legislation and professional standards.
